EASTON– March 18, 2015 – Delmarva Foundation, a nationally recognized leader in quality improvement and community engagement, announced today it will exhibit at the 15th Population Health Colloquium from March 23 – March 25 at the Loews Philadelphia Hotel, the leading forum on innovations in population health. Delmarva Foundation will showcase services and strategies particularly focused on community-based projects. The Population Health Center (PHC) at Delmarva Foundation bridges the gap between public health and clinical medicine concerns for community health needs and brings together concepts and skills from both arenas.
During this three-day event which involves leaders from academia, industry, and government. Visitors to the Delmarva Foundation booth will view demonstrations of the Resource Access Card (RAC)—a tool for participatory research. Community-based organizations can use the RAC card to collect data and store it in the cloud. This is an inexpensive and secure way to track the health habits of large groups. In addition, the Best Care Connector (BCC)—an innovative care coordination service, will be showcased. The program provides comprehensive access to data for small and large organizations involved in care coordination. The BCC uses the Pathways Community Hub model of care coordination, which is AHRQ approved, and provides tablet access with secure cloud data storage. Risk stratification and braided funding are unique components of this system. Visitors can also explore Baltimarket — an innovative model for providing health information to communities on both food access and health education and primary care resources.
The Population Health Colloquium brings together leading experts, who share the goal and responsibility to transform health care. The conference will focus on building a culture of health by providing real world examples of population based care being implemented by leading companies and organizations. It will feature nationally prominent experts and showcase innovative programs, initiatives and solutions.
